The AHA PPO Plan is designed to give you freedom and flexibility. You have the ability to visit your doctor of choice while the HMO offers coverage through a network of health care providers. When using a participating provider in the PPO plan, you receive greater benefits, while coverage is also available for out-of-network doctors and facilities.

With an HMO, you are required to select a primary care physician (PCP). Except in the case of medical emergency situations, you need a referral from the PCP in order to see a specialist or other doctor. The plan does not cover out-of-network benefits.
